1. Official High school transcript or GED certificate, transcripts graduation from college transcripts.

2. Neg TB Skin test

3. Able to pass drug screening and background check FELONY ON RECORD WILL RESULT IN A "NO" AT CLINICAL SITE.

4. (2) Forms of identification including Social Security Card, Government issued ID

5. Submit proof of Hepatitis B vaccine or signed vaccine declination form.
